Acceptance of Health Insurance
At Neighborhood Health Center, we are dedicated to getting you accessible health care. We accept most insurance plans and will work to add additional plans as the need arises.
If you are struggling to afford your medications, Neighborhood Health Center can assist with prescription drug costs with various programs and partnerships. Ask a team member how we can help you stay well.
Online Patient Portal
Neighborhood Health Center utilizes Epic as the electronic medical record. One significant advantage for our patients is the My Chart patient portal. It allows our patients to communicate with their provider, access test results, request prescription refills and manage appointments.
Patient Rights and Responsibilities
Neighborhood Health Center strives to provide our patients with the best possible quality of care and to ensure that our patient’s dignity and privacy are respected. We appreciate you trusting us with your care during the different phases of your life. As a patient, you have certain rights and responsibilities we want you to be aware of. Please call the Neighborhood Health Center Compliance Officer at 765-965-4299 to have your compliments and complaints heard and addressed.
Despite efforts to stay completely healthy, it is still possible to fall ill. When this happens, it can be quite a financial burden. Neighborhood Health Center has partnered with Claim Aid to assist patients with eligibility services. Claim Aid provides services for referred uninsured and underinsured (self-pay) patients. They screen for potential coverages for all categories of Health Coverage Programs governed by federal and state entities, the Health Insurance Marketplace (HealthCare.gov) as well as hospital financial assistance programs. With our financial aid, you and your loved ones will be able to see a provider whenever you need to.
Our mission is to promote health and wellness through access and affordability for all members of our community. As part of this mission, we also provide a sliding fee schedule to primary and behavioral health patients who are at or below 200% of poverty level.

Would you like to have better management of your chronic illness? With our care coordination program, you can. A designated care coordinator from Neighborhood Health Center will arrange for follow-up appointments after your hospitalization and will also attend these appointments with you to make sure that all parties involved (you, your doctor, and your caregiver) are fully informed and updated about the state of your health and your chronic disease. Your care coordinator will also educate you and your family on your chronic disease so all of you can manage it better and prevent avoidable hospitalizations in the future. This service is ideal for people dealing with:
Chronic obstructive pulmonary disease (COPD)
Renal (kidney) failure
Our care coordinators will work with you and your family to help you avoid hospital stays by better managing the condition through a healthy lifestyle.
