Primary Care for Our Neighbors

When you need a provider to look after you and your loved ones’ well-being, consider choosing a primary care provider (PCP) at Neighborhood Health Center. We encourage you to bring your family in for regular checkups. Preventative care is extremely important for diagnosing conditions early, and managing them so you or your family members can live comfortable and healthy lives. If you are worried about the financial aspect of getting health care, we have financial assistance programs available and can provide transportation assistance.

Primary Care

Primary Care Providers (PCP) are available to care for individuals and families of all ages. A PCP can be a doctor, physician assistant, or nurse practitioner. When you choose to see a PCP at Neighborhood Health Center, your care provider will be involved in your health on a long-term basis. Our primary care services encompass:

  • Family care (all ages)

  • Internal medicine (ages over 18)

  • Pediatrics (ages 18 and under)

  • Geriatrics (ages 60 and older)

  • Gynecology services

  • Telemedicine visits

Vaccinations

Whether you have children or are an adult planning to travel the world, vaccinations are a must for everyone. Vaccines are essential to protect family members who have weaker or compromised immune systems, as they are often unable to be vaccinated themselves. Vaccinations are very important in keeping us healthy. These injections enable our bodies to fight off diseases effectively, protecting us from serious illnesses. Examples of vaccinations we provide include:

  • COVID 19

  • Flu

  • Tetanus

  • Polio

  • Chicken pox

  • Shingles

  • Hepatitis A & B

  • Hib

  • HPV

  • Measles, Mumps & Rubella (MMR)

Lab Testing

When you have seen your provider and discussed your care, he or she may need more information to better diagnose or treat your illness. Laboratory testing may be needed to facilitate your care plan. Better information means more effective treatment. Our caring team will ensure you have the testing you need in order to make you well. NHC utilizes LabCorp as our testing partner. Together, we will ensure you are able to get the testing you need, regardless of cost.

You may be eligible for low or no cost laboratory services. We do not want cost to keep our patients from accessing lifesaving laboratory testing.

Referrals

Your PCP will provide you with “primary care,” which is providing preventative care and teaching healthy life choices, along with performing initial assessments when you have a health complaint. In the case that your issue is better suited for a specialist, we can refer you to a provider who can examine you further.
